Personal Contribution Lump Sum Deposit Form Make lump sum deposit at any time. Lump sum deposit can be made by direct debit if member has a regular personal contribution instruction.	INVT
Rebalance of Existing Portfolio Form Change the investment allocation of both existing account balance and any future contributions or amounts transferred into your account. We will redeem the necessary units to rebalance the portfolio of your existing account balance. For SimpleChoice Your portfolio rebalance can be performed only once per calendar year. Asset Switch of Existing Balance Form Switch all or some of the units from one fund or funds and then re-allocate the redeemed amount to another fund or across any of the funds. Scheme Member's Request for Fund Transfer Form Preserve your accrued benefits in the existing HSBC MPF scheme when you cease employment; Transfer your accrued benefits from another MPF scheme to an HSBC MPF scheme, or vice versa.	INPM
Statutory Declaration on Cessation of Employment Supporting document for employee's cessation of employment and the date of cessation. This form is applicable in the circumstances where: the trustee concerned is satisfied that the former employer cannot be located; or the employer concerned refuses to submit written notice to the trustee about the cessation of the employment of the employee. Statutory Declaration on reason on cessation of employment and Severance Payment / Long Service Payment and Transfer of Accrued Benefits with Outstanding Contributions Supporting document for employee's cessation of employment and the date of cessation. This form is applicable in the circumstances where: the employee concerned has accrued benefits attributable to the employer's voluntary contributions and/or ORSO transfer; or the employee concerned has completed at least two years of service with the employer. This form should be submitted together with the Statutory Declaration on Cessation of Employment (MPF(S)-C(SD)).
