To ensure accurate processing of your requests, always check the code and use the appropriate forms. You can refer to your enrolment confirmation notice to find out which scheme you are participating in and other details such as Scheme ID and MPF membership number.

Rebalance of Existing Portfolio Form For SuperTrust, SuperTrust Plus and ValueChoice Change the investment allocation of existing investments and any future amounts transferred into your account. We will redeem the necessary units to rebalance the portfolio of your existing investments. For SimpleChoice Change the investment allocation of existing investments and new transfer-in MPF Benefits and AVC Benefits. We will redeem the necessary units to rebalance the portfolio of your existing investments. Your portfolio rebalance can be performed only once per calendar year. Scheme Member's Request for Fund Transfer Form Transfer of accrued benefits from another MPF scheme to an HSBC MPF scheme, or vice versa. It is also applicable for employees who cease employment to transfer their benefits to an individual preserved account.	INPM
