Declaration

Premium and levy payments

  • You, as a policy applicant and payment account holder, have confirmed that you are not acting for any other person, and you have authorised The Hongkong and Shanghai Banking Corporation Limited ("HSBC") to debit from your specified account the premiums due as instructed by HSBC Life (International) Limited (“HSBC Life” or “we”) and any applicable levies on behalf of the Insurance Authority ("IA").

 

Customer information

  • You have confirmed you were in the Hong Kong SAR when making this application.
  • All statements and information you have provided in this application are complete, true and up-to-date. You will immediately notify us in writing if these become inaccurate before we issue the policy. You have understood we will approve or decline any future claims based on these statements and information.

 

Personal information collection and use

  • You have understood HSBC may request updated personal information and we may not issue the policy if you do not provide the requested information by the specified time.
  • You have understood that the personal information provided in this application will not automatically be updated in HSBC’s records and therefore you must make a separate request to change your information if required.
  • You have consented to HSBC providing your personal information to us so we can evaluate and process this application.
  • You have understood that we will use and disclose your personal data we currently or subsequently hold according to the Personal Information Collection Statement.

 

Application approval conditions

  • You have understood that submitting this application does not guarantee its approval and we will only approve the policy after (i) we are satisfied you have met our prevailing eligibility and underwriting requirements, and (ii) we have processed your first premium payment and applicable levies. If the application is approved, the policy will be effective from the submission date.

 

Cooling off period

  • You understand that you have the right to cancel the policy and obtain a refund of any premiums and levies paid, by giving a written notice to HSBC Life (International) Limited incorporated in Bermuda with limited liability. You understand that to exercise this right, the notice of cancellation must be signed by you and received directly by the office of HSBC Life (International) Limited at 18/F, Tower 1, HSBC Centre, 1 Sham Mong Road, Kowloon, Hong Kong during the cooling off period. You understand that the cooling off period is the period of 21 calendar days immediately following either the day of delivery of your policy or the day of delivery of the cooling off notice to you or your representative, whichever is earlier. You understand that the cooling off notice is a notice that will be issued to you or your representative to notify you of the cooling off period around the time the policy is delivered.

Child protection conditions


For the child protection,

  • you have understood your children, who are at the age of 18 or below, are not covered by the child protection, if he or she has been diagnosed with premature birth, postmature birth, pre-existing conditions and/or congenital disease or is awaiting or undergoing any medical advice, treatment or medical investigation.

 

Suitability and affordability declaration


For the HSBC Family Protector basic plan and/or accidental death benefit,

  • you have understood that HSBC Family Protector is a life insurance product with no savings element. It is designed to help you fulfil your objective for financial protection against adversities in case of death.
  • you have declared that for your entire premium payment term, you can afford the premium payments and applicable levies required which are less than 50% of your disposable income and/or liquid assets.

For the critical illness benefit or cancer benefit and/or hospital cash benefit and/or child protection,

  • you have understood that HSBC Family Protector is a life insurance product with no savings element. You have agreed your application of HSBC Family Protector, including the selected additional benefits and protection amounts, can help you fulfil your financial protection needs against adversities in case of death and prepare for the expenses incurred in unexpected medical scenarios, such as key critical illness protection or cancer protection and/or spending on hospitalisation for yourself, and/or the special protection for your children against death and any unexpected cancer and hospitalisation cost for your children.
  • you have declared that for your entire premium payment term, you can afford the premium payments and applicable levies required which are less than 50% of your disposable income and/or liquid assets.

 

Policy replacement disclosure and declaration

  • You have confirmed that you do not plan to use, nor are you using, the HSBC Family Protector which is being applied for now, to replace any other life insurance policies which you owned or still own, that give rise to policy replacement. You have understood that if you are purchasing this policy to replace another life insurance policy, you should seek professional advice to understand the associated risks and detrimental consequences of policy replacement.

 

Insurance statement and advice preference

If you have registered for Personal Internet Banking and:

  • You have or have had policies with HSBC Life, we will follow your latest insurance statement and advice preference.
  • You never have policies with HSBC Life before (i.e. the policy you are now applying for will be your first policy with HSBC Life), you will receive eStatements and eAdvices.

If you have not registered for Personal Internet Banking, you will receive paper statements and advices.
